At The Jefferson Health Plan, we are dedicated to protecting the confidentiality of information we have about plan participants and take that responsibility very seriously. In addition, federal laws require us to maintain the privacy of protected health information and to provide participants with this notice of our legal duties and privacy practices with respect to protected health information.
This Notice of Privacy Practices describes how The Jefferson Health Plan may use and disclose protected health information to facilitate treatment, carry out payment of health care providers, and for other purposes that are permitted or required by law. It also describes member rights to access and control individual health care information.
Protected health care information is:
• Information about individuals covered under a plan sponsored through The Jefferson Health Plan,
including demographic information that may identify members;
• Information that relates to individuals' past, present, or future physical or mental health conditions;
and
• Any health care services an individual covered under a plan may receive as well as any payments
made for those services.
Examples of protected health information include, but are not limited to, the notes doctors keep that document members' physician office visits, birth dates, and social security numbers.
The Jefferson Health Plan is required by law to abide by this Notice as long as the terms remain in effect. We reserve the right to change the terms of this Notice of Privacy Practices as necessary. Any revisions to this Notice will apply to all the protected health information we maintain, including protected information received before the change was made. Except when required by law, The Jefferson Health Plan will not implement a material change to any of the policies or terms described in this notice prior to the effective date of a new notice.
All members, enrolled with The Jefferson Health Plan at the time changes are made in this Notice, will receive the revised Notice 60 days prior to the effective date of the changes.
